What we take, and why.

When you start a file we collect your name, work email, mobile, title, company name and address, worksite state, eligible headcount, industry, what you already offer, a target contribution, and a hoped-for start month. If you check the box, we also store that you asked Matt Ragudo / A La Carte Health to email, call, and text you, plus the time and the page you saw.
We use that to open a benefits file and contact you. We do not sell it. We do not ask for employee names, dates of birth, SSNs, or health questions on this form. You can ask for what we have, or ask us to delete the file, at Hello@alacartehealth.com.
